To submit your claim quickly, independently and avoid postal costs and delays:

  1. Download and fill the form associated with your request.
  2. Use our secure electronic document filing website.

For a faster process, make sure you have the following information handy:  

  • Your insurance contract number  
  • Your health insurance number  
  • Your completed and signed claim form  
  • Original and detailed invoices  

Fill out all required fields* with accurate information to allow us to process your request quicker.  

After clicking Submit request, you will receive an automatic email notifying you that the documents have been sent successfully.
